Aminolevulinic Acid/Photdynamic Therapy (ALA/PDT) is a relatively new procedure offered for the treatment of moderate to severe cystic acne. In this treatment a photosensitizing agent, Levulan (Aminolevulinic Acid ? ALA), is applied to the skin. The Levulan penetrates the skin and accumulates in the acne skin cells and oil glands. The Levulan is then activated with a specific wavelength of light called blue light (also known as ClearLight). This process deactivates the bacteria that trigger acne, exfoliates the skin to unclog pores, and shrinks the sebaceous glands in the skin, the root cause in the formation of acne. The results are clearing of acne lesions, minimized pores and reduced skin oiliness. Patients will also see diminished older acne scars and overall improved skin texture. ALA/PDT is usually applied to the face but is also effective for treating acne on the chest or back.
ALA/PDT provides a viable alternative to Accutane, an acne prescription drug that can have a number of serious side effects, including birth defects.
The Process
After cleansing, Levulan is applied to the skin in the office and left to absorb for 45 to 90 minutes. The Levulan makes the skin very sensitive to light. The patient is then placed under the ClearLight for 6-15 minutes. This activates the Levulan. Stinging of the skin may occur during activation but should subside after the procedure. After treatment the patient must remain indoors and avoid all direct light for 24 hours, as the skin remains extremely light sensitive. Additionally, direct sunlight MUST be avoided for a week following treatment, otherwise burning and blistering may occur. Wear an SPF 30 or greater sunscreen with UVA coverage at all times, even indoors. The treated skin area will be red for two to seven days. Our Jane Iredale makeup will hide most of the redness. After 48-72 hours, the top layer of skin will scale, flake and peel. Temporary swelling around the eyes can occur for a few days. Typically the client returns to the office after two days for a hydrating facial.
For best results, acne patients usually require a series of five ALA/PDT treatments, each spaced two to four weeks apart.
